CSU Release of Liability/Waiver

Waiver of Liability, Promise not to Sue, Assumption of Risk, and Agreement to Pay Claims (Waiver) are legal forms under which one party waives the right to hold another party financially responsible for injuries and property damage. A properly administered waiver can protect the University and its employees from liability for injuries that occur to students or other individuals who participate in activities on and off campus. A waiver is a valid and reliable legal tool under California law and should be used to provide a level of protection to the University from liability for accidents and risks in certain circumstances. The University is responsible for conducting programs and activities in a manner that does not impose an unreasonable risk of loss or injury. Although these agreements are primarily legal tools, they also serve an educational purpose by making participants aware of potential risks. Often providing participants with this information is all that is necessary to avoid preventable accidents.

Sending a Waiver Using Adobe Workflow

Instructions for AdobeSign workflow: If you are coordinating a voluntary activity that requires a waiver, IRM recommends sending participants an email with clear instructions for completing the waiver. Template email to participants below:

Please click on the workflow link [enter link] and enter your email address in the Participant field. In the CC field enter [input email of the person responsible for collecting the form]. The workflow will then be sent to your email. Open the email link and complete the following fields:

  • Activity: [detailed activity description],
  • Location [insert location of activity], and
  • Date and time [insert exact date and time(s)].

Review the CSU Standard Release Waiver, sign and date on the bottom and submit.

Note: If using the Approved CSU Liability Waiver Minor make sure to enter the parent/legal guardian鈥檚 email address in the Participant field.

Group Participant Release PDF
Group waivers may be utilized for low to moderate risk activities where risks and conditions are uniform for all participants (e.g., class field trips, club outing) and when collecting individual waivers is impractical due to group size or activity. Consult with IRM if you have questions.

Note: Group waivers are not to be used for minors - minors require parent/guardian consent individually.
