Motor Vehicle Accident Information
Office of Insurance and Risk Management (ORIM): administers the state motor vehicle
auto liability insurance program. Integrated Risk Management will submit forms to ORIM on behalf of State employee.
Important note: If you get into a car or cart accident while driving for University
business, it could affect your driving record and result in points assigned by the
Department of Motor Vehicles (DMV).

Supervisor’s Review – for the supervisor to investigate each accident, report facts
and circumstances, confirm that the State vehicle was used on State business, and
initiate or recommend action to achieve accident prevention. |
Instructions for Motor Vehicle (including carts) Accidents while on University Business:
- Determine if you or anyone involved in the accident needs immediate medical assistance
and, if so, call 911.
- At the accident scene, make no comments or statements regarding the accident to anyone
except the investigating police authority, your supervisor, or ±¬ÁÏÉçÇø Integrated Risk
Management staff.
- For accidents occurring outside of incorporated areas, contact the California Highway
Patrol; for those within cities, contact the local police department or sheriff's
department. If on campus, report the accident to the University Police. If any police
agency responds, obtain a police report number.
- If you are driving a campus or state vehicle, there should be a Std 269 card in your
glove compartment, which you may use to write down as much information as you can.
Tear off the perforated part and give it to the other driver so that they will know
that you work for ±¬ÁÏÉçÇø and they will have our contact information.
- If safe to do so, take photographs at the accident scene to include damages to the
state vehicle, the other party’s or parties’ vehicle/property, etc.
- Report all accidents that occur while on University business immediately to your supervisor,
Safety Health & Sustainability and Integrated Risk Management within 48 hours.
- Complete the Std 270 form as soon as possible, and, if obtained, include the police
report number. Have your supervisor review the Std 270 form and sign it. Your supervisor
must also complete a Std 274 (State Driver Accident Review).
- Immediately send the original Std 270 and Std 274 to Facility Services (if a state-owned
vehicle) and Integrated Risk Management. If the accident occurred while driving a
personal or other non-state-owned vehicle, send the Std 270 and Std 274 only to Integrated
Risk Management.
If you are contacted by the other driver, their insurance company or their attorney,
refer them to Integrated Risk Management at 760-750-7475 and/or risk@csusm.edu. Do not discuss the accident with them or provide them with any statements regarding
the accident.
For Accidents That Involve Personal Vehicles:
- If you are driving a personal vehicle, refer to Instructions for Motor Vehicle Accidents while on University Business, above. It's important to know that the University's insurance only covers liability that
goes beyond what your personal auto insurance covers. In other words, your own insurance is the first line of coverage, and the University's
insurance would only apply if your liability exceeds your personal policy limits.
- Before you are authorized to use a privately owned vehicle to conduct University business,
you must certify in writing that the vehicle used will always be:
- Covered by liability insurance in at least the following amounts: $15,000 for personal
injury to, or death of, one person; $30,000 for personal injury to two or more persons
in one accident; and 5,000 for property damage;
- Adequate for the work to be performed;
- Equipped with safety belts in operating condition; and
- In safe mechanical condition as required by law.
This certification will be recorded on Std. Form 261, Authorization to Use Privately
Owned Vehicles on University or State business. See Defensive Driving Program Overview for more details.
For Accidents That Involve Rentals:
- If you are driving a rental vehicle while on University business, refer to Instructions for Motor Vehicle Accidents while on University Business, above, report the accident to the rental car provider and follow their instruction
for handling the vehicle.
For the University Employee Driver’s Immediate Supervisor:
- Review and co-sign the Vehicle Accident Report (Std 270) to attest to the fact that
the employee was on official state business.
- To the extent possible, investigate the accident to determine if it was preventable.
- Complete the Supervisor's Review of Motor Vehicle Accident form (Std 274).
- Send the original Std 270 form and Std 274 form to Facility Services – Fleet Management
(if a state-owned vehicle was involved) with a copy to Integrated Risk Management.
If you are the state driver, and you have been served with a Small Claims Court Summons,
Municipal or Superior Court Summons and Complaint, notify Integrated Risk Management
at 760-750-7475 as soon as possible.
